Before arriving on campus and before receiving medical care, Curry students must complete several steps including filling out important health history forms. These forms can be found on the patient portal, Pyramed.
Students should do the following:
- Complete the required Curry College required forms, found under the 'Forms' tab on the Pyramed patient portal.
- Upload the required documents. Documents can be uploaded via links embedded within the required forms in the 'Forms' tab or via the 'Document Upload' tab on the patient portal.
Required forms include:
- Curry College Medical History Form
- Curry College Immunization Form
- Curry College Insurance Plan Form
- Meningitis Waiver* - only complete this waiver form if you have not received Meningococcal ACYW vaccination on or after the age 16. Please review your immunization record to determine if you need to complete this waiver.
*This waiver is not intended for the Meningitis B vaccination as Meningitis B vaccination is not required.
Also Required
- Upload an official copy of Immunization record, this can be from your health care provider, your health portal, or your school records
- Upload a copy of health insurance card, both the front and back
- Students under the age of 18 at time of enrollment will need to have parent or guardian sign the Consent to Treat form, and uploaded to the Pyramed portal.
Recommended
- Upload any pertinent medical records, especially if you will be seen for a pre-existing medical condition.
- Upload your Medical Emergency Plan if you have one.
NOTE: Forms are due by July 15 - Students who do not complete these required forms/steps will have a “health hold” placed on their student account, which will prevent them from registering for classes.
How to Access the Patient Portal:
- Sign into myCurry Portal with your Curry email
- Choose the PyraMed icon
- Forms are found under the 'Forms' tab (top header)
- Documents can be uploaded under 'Document Upload' tab
